Welcome to Phoenix Kids Club,


Thank you for choosing the Phoenix Kids Club as your choice of care for your child. Please complete the following steps to complete enrollment:

 

Step 1:

Contact the facility to ensure space is available for your child(ren)'s age group.

Step 2:

If space is available, ask to schedule a tour.

​

Step 3:

Upon completion of the tour, visit our website at www.thephoenixkids.com to select your chosen membership option below.

Step 4:
Check your email

(Please check your SPAM folder just in case.)

Step 5:

Download and print the application for enrollment. This document can also be provided by your new facility director. Schedule your new member orientation.  Here you will fill out your Enrollment packet completely and schedule your child(ren)s start date.

 

Step 6:

If your brought your enrollment packet home, please return it with a copy of your child(ren)s up to date shot record, ear, eye, and dental forms, and a copy of your child(ren)'s birth certificate.
